This agreement confirms enrollment in the Milestone DOT Compliance Random Drug and Alcohol Consortium Program in accordance with Federal Motor Carrier Safety Administration regulations 49 CFR Part 40 and Part 382. Please initial each acknowledgment and sign at the bottom.
Section 1 — Client Acknowledgments (Initials Required)
I understand a pre-employment drug test or valid transfer documentation is required prior to entering the random pool.
I understand all enrolled drivers are subject to equal chance random selection generated by a compliant random selection system.
I understand a Designated Employer Representative is recommended to ensure timely notification and response to selections.
I understand upon notification of selection I must report immediately and arrive at the assigned testing location within the required reporting window.
I understand failure to report, refusal to test, or failure to complete testing will be reported as a refusal in accordance with federal regulations.
I understand removal from safety sensitive duties may occur until the return to duty process is completed when required by regulation.
I understand reinstatement after noncompliance may require additional administrative processing fees.
I understand failure to maintain payment for services may result in removal from the consortium and notification to the employer.
I understand enrollment fees are administrative and nonrefundable once enrollment has been processed.
I understand continuation of services requires renewal prior to the anniversary date.

Google Sheets Sync — Setup Instructions

Follow these steps to connect your admin portal to Google Sheets. This allows your driver pool and draw history to sync across devices. Estimated setup time: 15–20 minutes.

Create a New Google Sheet

Go to sheets.google.com and click the + button to create a new spreadsheet. Name it Milestone DOT Compliance — Admin Data. Create three tabs at the bottom: DriverPool, DrawHistory, and SAPDrivers.

Open Google Apps Script

In your Google Sheet, click ExtensionsApps Script. This opens the script editor. Delete any existing code in the editor.

Test the Connection
